Patrick said he didn't have a sister in Something Smells, though he could of said it out of stupidity and/or a sister was not thought of back then.
But, then, in this episode, he said nobody knew he HAD a sister, so he could have forgotten until the events of this episode, or the events of Something Smells take place in the future after Sam has died.
It's weird how Sam can very easily come to visit Patrick because she got lost even though the explanation for that is obvious.
Also, in the episode that Patrick is found to be king, his sister isn't shown on the medieval family tree chart. This may indicate that Sam is only Patrick's step-sister. Or because Pat said nobody knew he had a sister.
This episode refers to iCarly where Carly has a "sister" named Sam, but that is not her real sister, just a rumor from iCarly fans.
