Each reviewer has a column. Each column begins with the "N/A" rating:

Simple replace the "U" with your rating:

Make sure you put the rating in the correct column. On the bottom of the page, under reviewer notes, you can make notes, such as giving the date(s) in which you reviewed a certain group of articles.
